Subacute Care

After a hip or knee replacement, cardiac event, stroke, or other acute event, we understand that residents want to keep moving forward to gain and sustain momentum toward a return to an active and healthy life. Our goal is to make this happen as quickly and as safely as possible.

We offer skilled short-term rehab to assist those who, after a hospital stay, are in need of a sound rehabilitation program before returning home. Our comprehensive rehabilitation program includes physical therapy, occupational therapy, speech therapy, post-surgical care, IV therapy and a host of other rehab services that are designed to support the transition from hospitalization to home.

Each rehabilitation program is custom tailored to meet the unique needs of the individual with the goal of restoring function and help you reach the highest level of independence. Whether one needs to recover from recent surgery or to regain strength after an illness, our personalized rehab-to-home program at The Palace Care Center will help you achieve your goal. Our program offers a wellness-focused, approach to rehabilitation that includes the resident, the resident’s physician, therapy team, and family. Our combination of clinical expertise, state-of-the-art equipment, and dedicated and caring staff, will ensure that each resident returns home as soon as possible.

 On a daily basis your loved one will partake in recreational activities that stimulate the mind, body and soul. Each resident will have an individually customized plan-of-care that will be geared to maximizing life long passions and hobbies in order to restore and rejuvenate memory and function.
